This webinar addresses a clue that is easy to overlook in pediatric practice: gallbladder involvement is part of the metachromatic leukodystrophy (MLD) disease spectrum and may be present before neurological symptoms appear. When a gallbladder finding in a child has no clear common cause, it can point to an underlying systemic disorder. Led by Prof. Carlo Catassi and Dr. Salvatore Recupero, the session outlines how to recognize these findings, when to look beyond the usual differential, and when to seek specialist assessment.

The session moves from the gallbladder conditions pediatricians commonly encounter, to the characteristic findings and evidence in MLD, to a clinical case that shows in practice how an unexplained finding can lead to diagnosis.

Expert presentations cover:

  • Gallbladder disease in pediatric practice (Prof. Carlo Catassi): Common pediatric gallbladder conditions and contributing factors, their presentation across childhood, the role of ultrasound, and the findings that should prompt a broader differential, including unexplained wall thickening, multiple or enlarging polyps, and unusual combinations of findings.
  • MLD and its gallbladder manifestations (Dr. Salvatore Recupero): How gallbladder involvement fits within a disease predominantly recognized through neurological deterioration, the principal abnormalities associated with MLD, what current evidence shows about their frequency and timing, including findings before neurological onset, and the limitations of that evidence.
  • Case presentation and discussion (All speakers): A clinical case followed from presenting complaint and gallbladder findings to diagnosis, with a joint discussion of which features warranted further investigation, how suspected MLD is confirmed or excluded, and the implications for siblings.
  • What should the pediatrician do next? (All speakers): A practical recognize, assess, and refer approach, and why diagnosing MLD before neurological deterioration matters.

The session features:

  • Common pediatric gallbladder conditions and the findings that warrant a broader differential diagnosis
  • The principal gallbladder abnormalities associated with MLD, illustrated with ultrasound examples
  • Current evidence on gallbladder findings in neurologically presymptomatic children, and its limitations
  • How specialist investigation establishes or excludes MLD, and when to refer

Learnings

By the end of this webinar, participants will be able to:

  1. Recognize common pediatric gallbladder conditions and findings that warrant a broader differential diagnosis.
  2. Describe the principal gallbladder abnormalities associated with MLD and the evidence that they can precede neurological symptoms.
  3. Understand when to seek specialist assessment and how suspected MLD is investigated.
